Biliary Hamartoma (Von Meyenburg complexes) are often multiple small (<5mm) nodules scattered throughout the liver.   - Definition = composed of small intrahepatic bile ducts, ductules, inflammatory cells, and fibrosis.   - Imaging characteristics = Characterized by numerous tiny (<5 mm) cystic lesions scattered throughout the liver . Lesions are T2 hyperintense, T1 hypointense, and do not enhance after contrast administration. They do not communicate with the biliary tree and remain stable over time, helping distinguish them from metastases or microabscesses.  Special considerations =   - Can be incorrectly categorized as metastatic carcinoma on frozen sections.   - Common in patients with congential hepatic fibrosis or polycystic liver disease.  Treatment & Surveillance =  - reassurance. If imaging or clinical concerns are present, consider excisional biopsy.
